- Your referral letter
- ALL x-rays, scans and tests relevant to your visit
- Your Medicare card and health fund details
- Your insurance or workers compensation information if relevant
- List of medications if appropriate

Consultation is by appointment only. Appointments can be made by phoning the surgeons rooms directly on the numbers listed on this page.
Appointments cannot be made via email or via this website.
Referrals
Please consult your General Practitioner before arranging an appointment with any Specialist. Patients wishing to claim from an insurance company (Medicare, Private Health Funds, Workers Compensation Insurer) must be referred by a GP. Please bring your referral letter to your consultation.
Medical Advice
Specific medical problems needs to be discussed with a surgeon in person so that medical advice is tailored for the specific needs of the individual. For this reason, medical advice can not be given via email. If you require a medical certificate please inform staff on your arrival.
